Job Description:

This position integrates the pharmacist as a member of the care team within the physician practice. The pharmacist is responsible to manage patients’ drug therapy to ensure optimal, cost effective care delivery, improve health outcomes and reduce provider burden. This role supports the full physician panel, regardless of health plan and may involve multiple clinic locations. Patient and provider engagement is both face-to-face and virtual.

Job Responsibilities:

  • Optimization of patient drug coverage benefits for appropriate care and reduced costs (preferred formulary products, lower cost alternatives, mail order and assistance programs)
  • Self-directed and must be able to manage multiple tasks and priorities in partnership with clinic leadership (physician, care coach, clinic administration)
  • Chronic disease state education for patients on appropriate use of medications and self-management
  • Execute clinical protocols in agreement with provider practice and as allowed per state regulatory agencies
  • Patient and provider consultation in the management of chronic disease states
  • Daily engagement (face to face/virtual): patients, physicians, nurse practitioners, nurses, medical assistants, social workers, clinic & market management and administrative staff
  • Provide post-discharge medication reconciliation per transition of care management protocol
  • Vaccine need assessment and education (administration if needed)
  • Comprehensive oversight of patient drug therapy
  • Participation in clinic team meetings (daily and monthly) and treatment plan development
  • Physician and clinic staff education for drug therapy and drug coverage updates

Job Requirements:

  • Strong attention to detail; able to concentrate without distraction in a fast-paced environment
  • Build trust and confidence with providers and patients
  • Understanding of managed care concepts and application in clinical practice
  • Excellent organization and communication (verbal and written) skills
  • Demonstrate initiative and accountability in execution of patient treatment plans
  • 2+ years’ experience as a practicing pharmacist
  • Ability to work in a team environment and support common goals
  • Familiarity with Medicare drug programs and quality requirements
  • Experience in managing drug therapy for patients with chronic conditions
  • Competent at the intermediate level or higher with MS Office programs such as Word, Excel and Power Point
  • Ability to prioritize multiple tasks, anticipate and solve problems
  • Certified in basic life support (BLS) and immunization administration or complete upon hire
  • Familiarity with electronic health record (EHR) systems
  • 2+ years working in an ambulatory care/primary practice setting
  • Clinical residency in ambulatory care or similar discipline
  • Certified diabetes educator
  • Multilingual ability preferred (may be required depending upon patient population)
  • Board certification in geriatric pharmacy
  • Identify and implement innovative solutions for improved efficiencies and outcomes
