Job Description: This position will serve as a patient advocate for all Specialty Pharmacy patients. The Specialty Patient Care Coordinator will help the patient navigate through the complex journey which entails both clinical support and financial assistance. Specialty Pharmacy prescriptions require a level of care and attention above and beyond non-specialty prescriptions. These prescriptions are generally very expensive, may require special handling and have specific administration requirements. The Specialty Patient Care Coordinator will work with the patient, prescriber, payer and the Specialty Pharmacy team to ensure the patient receives their medication in a timely fashion and has all of the support required to achieve the desired therapeutic outcome. Job Responsibilities: Responsible for entering the patient information including name, date of birth, insurance information, allergies and medication history into our therapy-specific patient assessment software (TherigySTM) Responsible for providing phone and on-line support for patients, prescribers, insurance companies and retail pharmacies, ensuring a specialty pharmacy patient gets and takes their medicine Responsible for monthly follow-up calls to patients to assess compliance with drug therapy, identify any barriers to patient adherence (for example, the patient cannot pick up the medication because it is too expensive or the patient feels like he/she has too many medications to take, or the patient can’t remember to take medications) and facilitate discussions with the Clinical staff to help the patient overcome barriers and process refills.
